Our goal is to provide affordable, exceptional care to our patients.
We are a direct pay practice; complete payment is expected at the time of service. If you have insurance, you can file for reimbursement.
We have negotiated with our laboratory and supply companies for the lowest rates possible and are passing that savings on to our patients. With lab tests, you pay us directly for all lab work at the time of your visit.
We provide several employee benefits healthcare package pay options for you to choose from –
1. Pay per visit. When you are seen, you pay an office visit charge plus the cost of each individual lab and/or procedure. First-time patient visit is $125, $99 for existing patients, plus the cost of labs and/or procedures. House-calls are $300 plus labs and/or procedures. Functional Medicine visits are $275.
$79 charge for phone / telemed consultations (free with Smart Care Plan).
Most of our basic labs/diagnostic tests* cost between $15 and $35.
- Basic Smart Care Plan You pay an enrollment fee (waived if pay in full for the year) plus a monthly fee and all office visits and basic labs/diagnostic tests* are free. Also includes a reduced housecall fee.
$69 a month for 12 months plus an enrollment fee of $99.
(Pay 1 year in full – $828 and the enrollment fee is waived plus receive a Complete Comprehensive Physical Exam worth $500 free** including basic labs/diagnostic tests, plus PSA or PAP Test and EKG)
Reduced housecall fee of $150 + procedures (basic labs/diagnostic tests* are included in house call fee).
- Premium Smart Care Plan You pay an enrollment fee (waived if paid in full for the year) plus a monthly fee and all office visits and basic labs/diagnostic tests* are free. Also includes 2 free house-calls a year and reduced housecall fee after the initial two free.
You also have direct access to your provider’s cell phone 24 hours a day/7 days a week. Have you ever been sick or had a sick child after hours and wish you had someone you could call? Want the convenience and privacy of housecalls any time of the day or night and even on weekends? Just want someone to ask to know if an emergency room visit is necessary?
$125 a month for 12 months plus an enrollment fee of $99.

We accept Cash, Check, Debt, VISA, Master Card, American Express and HSA Cards. $35 fee for all returned checks.
*Most basic labs/diagnostic tests fees are between $15 and $35. Basic labs/diagnostic tests include: CBC, BMP, Lipid Panel, and Urinalysis.
**Free Complete Comprehensive Physical Exam includes:
- PSA or PAP Test and EKG. Height, Weight, BMI, Pulse, Blood Pressure, Temperature, etc.
- Health Risk Factors: Diet, Exercise, Safety, Alcohol, Contraception, etc.
- Disease Prevention: Stroke, Coronary Disease, Cancer, Illness, etc.
- Physical Exam
- Health Maintenance: Review Immunizations.
- Lab Tests: CBC, BMP, Lipid Panel, PSA, Urinalysis, basic PAP and EKG.
- Recommendations/Referral
